There is a high prevalence of health issues in children living in foster care.
The DHS-945, Financial Aid Verification Form, shows that a student was in foster care after the 13th birthday, therefore allowing them to apply as “Independent” on the Free Application for Federal Student Aid (FAFSA). The form must be completed by a Department of Health and Human Services (DHHS) case worker or supervisor
